Completion of a clinical evaluation does not establish an ongoing provider-patient relationship or guarantee acceptance into the practice for ongoing treatment.

🤝 Collaboration With Your Clinician

Patients currently working with another clinician are encouraged to share their results of the clinical evaluation analysis with their treating provider for discussion and potential integration into their care.

🧬 Pharmacogenomic Testing Included

Pharmacogenomic testing is included in the evaluation fee for individuals who have not previously completed testing. If pharmacogenomic testing has already been completed within the past five years, existing results may be reviewed and integrated into the report. In these cases, a reduced evaluation fee may apply.

This is a self-pay consultation service. Insurance typically does not cover consultative evaluations or written analysis reports. Laboratory orders will be provided, and you may complete them at a facility of your choice; most patients are able to use their insurance for lab services, though coverage varies by plan.
